Eligibility
Eligible Employees
You may enroll in the Pyramid Healthcare Benefits Program if you are an employee working at least 30 hours per week.
If you are newly hired by Pyramid Healthcare, you will be eligible for benefits on the first day of the month following 60 days of employment.
Eligible Dependents
If you are eligible for benefits, you may also enroll your eligible dependents. In general, eligible dependents include:
- Your spouse
- Your dependent children up to age 26
- A child with a mental or physical disability whose coverage may continue beyond age 26, provided the child meets the plan’s requirements and proof of the ongoing disability is provided.
Proof of dependent eligibility or disability may be required.
When Coverage Begins:
For the 2026–2027 plan year, your benefits are effective August 1, 2026 – July 31,2027.
For newly hired employees who meet the eligibility requirements, coverage begins on the first day of the month following 60 days of employment.
Important: Benefit elections must be completed before your coverage effective date. If you do not complete your benefit elections by the required deadline, you may not have coverage in place when you become eligible.
Once elected, your benefit elections generally remain in effect for the entire plan year. You may change your elections during the next Open Enrollment period or if you experience a qualifying life event that permits a change under the plan.
Open Enrollment
Benefits are effective August 1, 2026 - July 31, 2027
Our Annual Open Enrollment period is held in June each year. Open Enrollment is your once-a-year opportunity to pick your plans or make changes to your existing coverage. The plan year begins August 1st and remains in effect through July 31st. All benefit election are in effect for the entire plan year and can only be changed during the Open Enrollment period unless you experience a Qualifying Life Event.

New Employees

All elections are in effect for the entire plan year and can only be changed during Open Enrollment unless you experience a Qualifying Life Event.
Qualifying Life Event
Certain life events provide you a 30-day window to elect benefits or change your covered dependents.
What is a Qualifying Life Event? If you experience any of the scenarios below, you may be eligible to update your benefit elections mid-year:
- Marriage, divorce, or legal separation
- Birth or adoption of a child
- Commencement or termination of adoption proceedings
- Change in child’s dependent status
- Death of a dependent
- Loss or gain of other health coverage for you and/or dependents
- Change in employment status
- Change in Medicaid/Medicare eligibility for you or a dependent
- Receipt of a Qualified Medical Child Support Order
Remember, dependent verification will be required.
If such a change occurs, you must make the changes to your benefits within 30 days of the event date. Documentation may be required to verify your change of status. Failure to request a change of status within 30 days of the event may result in your having to wait until the next open enrollment period to make your change.
